In order for ORACCA to process and to provide you a with a Brazing Certification replacement card you need to fill out the form below and choose a reason why a replacement card is needed.
This form will be your reason explaining why we are to issue you a new card and will be kept on file.
When all fields are filled out, Click "Submit" and you will taken to a success page that will contain a purchase button for card replacement. Fee is $35.00 and is non-refundable.
